Gauteng Legislature Raises Concern Over Mental Healthcare Failures After Health Ombud Report

The Gauteng Provincial Legislature’s Portfolio Committee on Health and Wellness has expressed serious concern following the findings of the Health Ombud’s report into the care and death of Ms Mohlamme at Dr George Mukhari Academic Hospital.

The report highlights critical systemic failures in mental healthcare delivery, patient safety, and governance within the provincial health system. The Committee has extended its condolences to the family of Ms Mohlamme.

Serious Systemic Failures Identified

According to the report, the tragic incident occurred in June 2024 during a transitional period following the national and provincial elections. The findings point to deep-rooted challenges in the management and delivery of mental health services in Gauteng.

Committee to Engage Key Stakeholders

As part of its oversight responsibilities, the Committee will first engage the Health Ombud to gain a comprehensive understanding of the report and its recommendations. The Gauteng Department of Health will also be called to participate in discussions aimed at ensuring transparency and accountability in implementing corrective measures.

Leadership Response

Chairperson of the Portfolio Committee on Health and Wellness, Ezra Letsoalo, emphasized the seriousness of the findings:

“We take these findings seriously and will ensure that they are fully interrogated. Our focus is on strengthening accountability and improving the safety and quality of mental healthcare services in Gauteng.”

Ongoing Oversight and Interventions

The Committee noted that the Health Ombud’s findings align with its ongoing oversight work, including a focused intervention study on mental health services across the province that began in November 2025.

This initiative has already included site visits to Dr George Mukhari Academic Hospital and several other healthcare facilities, forming part of broader efforts to address systemic challenges and improve patient outcomes.

Commitment to Mental Healthcare Reform

The Committee has reassured the public that it remains fully committed to addressing the issues raised in the report and ensuring that meaningful action is taken.

Authorities say the implementation of the Health Ombud’s recommendations will be critical in strengthening governance, improving patient safety, and restoring confidence in Gauteng’s mental healthcare system.
